Insurance Claim & Incident Form

This form is for reporting and making insurance claims following an incident related to a TMU activity.

As soon as you engage with other organizations for rentals of space or equipment, they will request that you provide proof of insurance. As Creative School students, you are covered by multiple policies that meet the vast majority of your insurance needs.

Liability insurance may be required if you are renting a location space and/or renting equipment from an outside source. If you are not sure if a certificate is required, check with the property owner or management in advance. You do not need an insurance certificate to record on Toronto Metropolitan University property.

TMU has production liability insurance that covers most student film, television, and media productions. The university has added an additional insurance policy that covers production-related risks that may not be included under TMU’s standard liability insurance. This includes productions that involve external participants, such as:

  • Hired actors
  • Intimacy coordinators
  • Performers, models, or other non-TMU participants

This insurance is set up as a blanket policy, which means individual student productions do not need to be registered and you do not need to submit a list of crew members or participants for standard projects.

Advance approval is required if your production involves higher-risk activities, including:

  • Stunts
  • Pyrotechnics or fire-based special effects
  • Watercraft (boats, jet skis, etc.)

If your production includes any of these activities, additional insurance approval is required before filming begins.

You will need to complete a risk questionnaire (links below), which must be submitted to the insurance provider for review and approval.

If your production involves stunts, pyrotechnics, or watercraft, please allow as much advance notice as possible, ideally 15–30 days before filming. This allows enough time for the insurance provider to review the request. Policy #5ET246S5G needs to be added to the questionnaire forms below.

Locations

Every Toronto Metropolitan University student is covered by a general liability policy. You can learn more about what is covered at the insurance policies webpage. If you need to provide proof of general liability insurance when arranging location use off campus, it can be provided in

the form of a certificate to the owners or supervisors of a location that you will be using (for shooting/recording, installations, etc.).  To get this proof of insurance, please fill out the Request

Liability Insurance Certificate and click submit. If you have questions about the form, please send your enquiries to insurance@torontomu.ca.

Our insurance contacts are very helpful people, but they need complete and accurate information as well as an appropriate amount of lead time to allow for processing. It may take up to 7 days for a certificate to be initiated, so please make sure your requests are filled out completely and that they’re made as early as possible.  

All faculty, staff and students of the university community are covered by insurance for the use of campus spaces for curricular purposes. No additional insurance is required.

If you want to use a university space for production you should inquire as to whether or not you need a permit.  More information can be found at space and event reservations..

Equipment

Student groups who require a (COI) Certificate of Insurance for renting equipment from third parties must send an email to certificaterequestscanada@marsh.com. Follow these steps:

NOTE:

If you are travelling, please check the countries with jurisdictional restrictions. If you travel to any of these countries, your insurance will not be valid.Jurisdictional Restrictions. Any province in Canada or state in the U.S. is totally fine. You would need the same information we typically request when students rent from local rental houses.

The equipment used by students in The Creative School available from the Equipment Distribution Centres (EDC/TRC) is also covered by the student policy #141-0084, that provides each student coverage when they are borrowing equipment from the school.  The policy has a $2,500.00 deductible. This insurance policy covers costs related to damaged or lost equipment taken out from the EDC/TRC valued between $2500.00 and $250,000.00. That means if equipment is lost or damaged while under your name, you will be responsible for paying the deductible of $2500.00. Please note that theft does not excuse your responsibility for the deductible of $2500.00, so take every effort to keep the equipment safe and secure at all times.  

Before you can take any equipment from the EDC/TRC you must fill out the appropriate liability forms.
